Life4ward, LLC (“Life4ward” “we” “us”) operates an informational website and web application located at www.onetrackhealth.com as well as a related mobile application linking to this Privacy Policy (collectively, the “Website” or “OneTrackHealth”) that, in addition to providing general information on chronic kidney disease, enables you through the OneTrackHealth mobile and web application to track your blood test results that you submit over time.

This Privacy Policy describes how Life4ward collects, uses, and shares personal information in relation to our Website and the test result management and other functionality its mobile and web applications provide (the “Service”).  Personal information means information that identifies you or that is or can reasonably be associated with you.

We collect personal information that you provide to us and technical information automatically when you visit and interact with the Website.

To create an account in order to use the Service, we collect your name and email address.  We also collect the diagnostic test results you choose to submit to the Service and the low and high test result parameters you set if you customize them. The Website may generate alerts and other reports when your submitted test results fall below or above your low and high set points.  We may collect other information you voluntarily submit to us, such as when you sign up for our newsletter, complete a survey, or email us.

When we delete personal information from your account (or delete your account entirely), we permanently delete your data from the Website’s active user database.  The data will thereafter remain in backups of the database until cycled out in accordance with our backup retention practices. You should be aware that traces of data can be recorded in multiple places in an information technology system and therefore it may not be technologically possible to remove each and every record of the information you have provided to us from our servers.
